^ Mobile Application Manager is included with the Packetstream Gold plan.  This is a limited time offer expiring December 31, 2004. Rates are charged PER MONTH PER PHONE.  Requires new activation, two-year service agreement and credit approval. $200 early termination fee applies. Digital cellular overage is $0.40 per minute and Nextel Direct Connect overage is $0.15 per minute, unless otherwise noted. Nights are Mon-Fri., 9:00pm - 7:00am. Weekend begins Fri. at 9:00pm and ends Mon. at 7:00am. Unused minutes do not accumulate to the next billing cycle.  Voice Mail and Caller ID charge of $1.00 each per month, unless included in a specific rate plan. Cellular minutes round to the next full minute. 

* For International Data Services, Express Messaging users pay $0.10/message plus per kilobyte charge.

**Nextel also imposes a Federal Programs Cost Recovery (FPCR) fee of $1.55 or $2.83. The FPCR is not a tax or government required charge. The fee is charged for one or more of the following: E911, number pooling and wireless number portability.   Additional charges may apply and may vary by market, including state and federal taxes, a Universal Service Assessment of either 1.124% or 1.25%, in some states a Gross Receipt Recovery fee of 1.4% to 5%, a TRS charge of approx. .07%, and a state-required E911 fee.
